Patient Online Services
We now offer an online service for our patients so you can book your appointments, order your repeat prescriptions and view a summary or detailed account of your medical record online at your convenience
Online appointment booking
Have the flexibility to book and cancel your appointments from home, at work or any location with internet access. You don’t need to queue at the practice, wait on the telephone and you can manage your appointments outside practice opening hours.
Request your repeat prescriptions online
Request your repeat prescriptions quickly online by logging into your account and simply ticking the appropriate boxes. You can review the progress of your repeat prescriptions and any messages that the practice may have sent to you.
Access to your GP Record online
Take greater control of your health and wellbeing by being able to view either a summary of your medication history, allergies and adverse reactions or a detailed account of your medical record which will include Medical problems, diagnosis and procedures, medication history, examinations and details of referrals.
In order to register for Online Services, you will need to request a registration form and patient information leaflet which are available from reception or click on this link to download the Patient Services Registrations form now. Once the form has been completed you will need to bring it to the surgery along with two forms of ID (one photo ID and one address ID). You will then be sent unique login details. Once you have completed your registration you will be able to book your Doctor's appointments online without having to telephone the surgery, request your repeat medications online and view parts of your medical record.
Accepted Forms of Identification
You will need to bring two forms of identification, one being a photo ID, the other verifying your address, along with the completed registration form.
We will accept any of the following:
- Photo card Driving License
- Photo Travel Card
- Student ID Card
- Utility bills (water, gas or electricity dated within the last 3 months)
- Bank/Financial Statement (dated within the last 3 months)
- Council Tax Bill
- Tax Documentation
Please note we can not accept Mobile Telehone Bills as proof of address
